children and families, apply today! Job Summary: To provide direct
services to individuals and families with complex trauma, abuse or
neglects, and intimate partner violence issues with the goal of
increasing stabilization, safety and well-being. Direct services
include assessments, intervention education, and linkage with
needed community-based resources and services. Duties and
Responsibilities: · Assess and refer children/individuals/families
to supportive and concrete services, housing, financial assistance,
education, substance use, mental health, medical, immigration,
legal, etc. · Provide case planning, assessment, crisis
intervention, establish goals and objectives and counseling
services for families who have a child at risk of Foster Care
placement. · Conduct assessments of families within 30 days of
intake and reviewed review and update in 6 months after intake. ·
Participate and attend all ACS, OCFS, Agency and Model Developer
training as requested or required. · Provide crisis intervention
and counseling services to children, teens, individuals and
families. · Collaborate with program case aid/advocate in aiding
and supporting families in obtaining resources, support services,
and advocacy assistance. · Collaborate with ACS to monitor
compliance with mandated services and family attendance of court
hearings. · Collaborate with ACS legal team to ensure all service
needs are being addressed by families. · Document and maintains all
mandated tracking logs/excel sheets, case record electronic
systems, and document in a timely manner. · Complete all program
required paperwork, i.e., Progress Notes, Program Family
Assessments, Family Assessment Service Plans (FASPs), and other
reporting requirements. · Request, participate, facilitate and or
attend conferences, meetings internal or external to the agency. ·
Assist individuals and families in developing services plans
addressing safety, stabilization, substance use, mental health,
domestic violence, from a harm reduction approach. · Participate in
weekly Supervision, with a planned agenda on family case issues to
discuss· Implement Quality Assurance data to drive case practice
and improve performance and achieve outcomes. · Monitor and obtain
information on the child/ren school placement and educational
performance. · Monitor and obtain information on the family’s
medical needs, including compliance with periodic physicals and
immunizations. Requirements: Skills, Knowledge, and Abilities: ·
Ability to use communication skills that successfully convey and
obtain accurate and complete information in a respectful, caring
manner. · Ability to communicate effectively with supervisors,
co-workers, and other community partners · Demonstrates the ability
to organize and coordinate work within schedule constraints and
handle emergent requirements in a timely manner. · Ability to
conduct interviews with families to obtain and provide the
information needed to carry out case management responsibilities ·
Knowledge and ability to use critical thinking and analytical
skills used to formulate with the family person achievable plans
for implementation · Ability to perform the essential functions of
the job with or without reasonable accommodation. · Collaboration
and facilitation skills to work with the individual, families and
services providers. · Demonstrates cultural sensitivity and
competence in work with families from various ethnic and cultural
groups, social-economic status and lifestyles.
Qualifications/Requirements: · Bachelor’s required. · Bi-Lingual is
preferred but not needed. Spanish, Chinese/Mandarin, Russian,
Italian, French Creole. · Experience with Microsoft Word, Excel,
PowerPoint, CNNX, PROMIS · At least two years of child welfare and
counseling experience preferred. · Due to the nature of the work
environment work hours will be based on the needs of program and
may vary · Perform other duties/responsibilities as assigned within
