Miller Memorial Community , Miller Memorial Community, a home-like, 90- bed residential health care facility with a low medicare census , seeks a qualified and experienced Registered Nurse for the position as the MDS Coordinator. Ideal candidates must have a current RN license in CT and must have at least 2 years previous experience in MDS. PDPM knowledge is a plus. Responsibilities include: coordination and planning of the care plan, MDS, and revenue enhancement maximizing RUG scores, admission, and change of condition. AA/EOE Job Requirements:* Coordinate care processes of resident assessments* Participating in resident care plan meetings* Participating in resident care plan* Provide general nursing care to residents in retirement and nursing community* Complete the resident assessment protocol for care planning* Assisting in delivery of resident care* Giving particular emphasis on nursing service plans of care for residents within the nursing facility* Complete comprehensive assessments and develop care plans for residents* Lead discussion and record resident care plans* Ensure that quality resident care is provided* Alter the current care plan of the resident* Developing a comprehensive care plan for each resident* Monitor quarterly and annual resident assessments and care plan reviews* Ensure that all personnel involved in providing care to the resident are aware of the resident's care plan and that nursing personnel refer to the resident's care plan* Optimize clinical and rehabilitation outcomes* Manage the care plan process* Prepare and direct care plan meetings for all residents in skilled nursing facility* Assess potential resident admissions to skilled nursing facility* Evaluate resident care plan for comprehensiveness and individuality* Coordinating resident's monthly summary and care plan goals to provide a continuum of nursing care
