DescriptionTo deliver professional nursing care to office practice
clients and their families. RN's work within the multidisciplinary
team using a holistic approach which involves the treatment of
responses to actual and/or potential health problems through case
findings, health teaching, counseling, and provision of care,
supportive to health promotion and prevention interventions to
attain optimal levels of health.Specific Duties And
ResponsibilitiesThe Staff Nurse is charged with and responsible
for:
- Ensures compliance with National Patient Safety Goals (NPSG):
patient identification, communication, medication management,
infection control i.e. hand hygiene, etc.
- Conducts initial evaluation/reevaluation and develop individual
care plan based on identified needs, treatment goals by using
indicators (labs, scores, symptoms) to determine progress.
- Updates and documents observation, medications, and other
pertinent new findings related to the care of the client.
- Administers vaccines and other medications from formulary as
defined in the Non-Patient Specific Orders (NPSO)
- Assists with diagnostic tests/procedures according to policy;
evaluate patient responses and make appropriate revisions in the
patient's plan of care.
- Recognizes and responds to emergency situations and act
effectively according to protocols.
- Assumes personal responsibility for safe work habits and
patient safety; utilizes/enforces standard precautions.
- Motivate, listen, coach and interact with the chronic disease
patient/family to set and achieve their self-management goal.
- Supervises nursing license staff.
- Performs unit management activities in absence of HN or as
assigned.
- Engages in the Patient Centered Medical Home (PCMH) model
collaborating with the patient and the care team focusing on the
goals and objectives for the promotion of optimum health,
preventive care and self-management.
- Actively participates in and maintains a state of readiness for
TJC.
- Participates in Performance Improvement and Quality Assurance
activities related to safety and satisfaction.
- Reviews daily In Basket messages
- Reviews and prepares pre-visit planning 2 -3 days prior to the
visits for chronic disease and preventive measures, immunization
status, labs, etc.
- Conduct daily huddles with all team members
- Provides client specific health promotion, disease prevention
and or management services to clients based on age, developmental
stage, family history, medical condition and ethnicity.
- Maintains compliance with HIPPA.
- Promotes and participates in community outreach programs to
raise awareness of services.
- Performs procedures such as electrogram, obtaining venous blood
specimens, POCs: HgbA1C, urine dipstick, pregnancy test, and blood
glucose with communication of abnormal finding directly to
provider, as indicated.
- Maintains professional competence through participation in
continuing educational activities, internal and external; maintains
mandatory licensure and certification such as BCLS.
- Adheres to Customer service standards treats staff, physicians,
visitors and clients with dignity and respect. ICAREMinimum
Qualifications
- A valid New York State license and current registration to
practice as a Registered Professional Nurse in New York State;
and
- Holds, or obtains through facility orientation, a valid and
current certification in Basic Life Support (BLS) through the
American Heart Association (AHA).Special NoteFor assignments in
Post-Acute Care ONLY, individuals meeting the following criteria
may also be considered for appointment: Graduation from a nursing
education program registered by the New York State Education
Department (NYSED) as licensure qualifying for Registered
Professional Nurse (RN) and has applied for licensure as an RN.
These unlicensed individuals may be employed to practice under the
immediate and personal supervision of a NYS licensed, currently
registered, RN for up to 90 days immediately following graduation
pending receipt of a limited permit, which selected candidates must
apply for and receive within the same 90-day period. Once a limited
permit is received, individuals can practice nursing, under the
immediate and personal supervision of a NYS licensed, currently
registered, RN at a NYC Health + Hospitals health care setting for
up to one (1) year from the date of issuance of the limited permit.
In addition, must hold or obtain through facility orientation, a
valid and current certification in Basic Life Support (BLS) through
the American Heart Association (AHA).Department Preferences
- Has knowledge and understanding of chronic diseases across the
adult age spectrum, including geriatric population.
- Demonstrates strong and positive interpersonal and customer
service skills
- Demonstrates effective communication skills both oral and
written.
- Exhibits the ability to establish successful interpersonal
relationship among diverse population regardless of ethnicity,
