general supervision of the Head Nurse, will provide direct care to
patients and delegate care of patients, based on qualifications of
the staff, in the absence of the Head Nurse.Areas Of

- Patient Care AssociatesSummary Of Duties And Responsibilities
- Supervises, educates, orients and makes patient care
assignments for professional and non-professional staff based on
acuity, workload and staff competencies/skill level.
- Assist the Head Nurse in directing staff in nursing care
activities in the assigned practice.
- Assumes team leader role for a care team in their
practice.
- Provides direct care to patients (see Staff Nurse job
description).
- Identifies care needs for the patient populations at the
practice level and works in conjunction with the leadership team of
the practice to develop/implement new models of care to improve
patient outcomes.
- Records activities, documents assessments, plan of care,
interventions, evaluations and re evaluations of patient status,
and education in the Electronic Medical Record.
- Administers medications, vaccines and/or treatments in
accordance with nursing policies and documents in the Electronic
Medical Record.
- Implements the nursing process utilizing assessment skills and
strategies to plan for the care of the patient based on identified
needs, problems or provider's orders.
- Reports all Communicable Diseases/Conditions to the Department
of Health using the on line submission service.
- Knowledgeable in Infection Control Procedure and techniques as
well as Patient Safety.
- Delegates tasks to unlicensed health care providers. Performs
POC testing, as necessary, and record the results in the Electronic
Medical Record.
- Acts as a preceptor for nursing students and supports the
development of other staff; acts as a resource.
- Assumes responsibility for own professional growth and
maintains competence through participation in continuing and higher
education programs.
- Able to communicate effectively with all members of the
healthcare team.
- Objectively evaluates and documents performance of personnel
and takes corrective action as appropriate.
- Participate in facility wide committees and projects as
needed.
- Participate in nursing quality improvement projects and
monitor/sustain gains of the activities.
- Active role in orientation, coaching/mentoring, performance
evaluations and disciplinary actions of nursing staff.
- Identifies learning needs of staff and assist with development
of in-services to address knowledge gaps.
- Assist with daily scheduling, assignments of nursing staff in
the practice.
- Directs the daily flow of the clinic making changes in the
staffing assignments as needed to ensure safety and on time
performance.
- Conducts/participates in daily briefings and huddles.
- Responsible for the nursing activities in the practice in the
absence of the Head Nurse.
- Performs assigned responsibilities as requested.Minimum
Qualifications
- A valid New York State license and current registration to
practice as a Registered Professional Nurse in New York State;
and
- Holds, or obtains through facility orientation, a valid and
current certification in Basic Life Support (BLS) through the
American Heart Association (AHA); and
- Six (6) months of experience as a registered nurse.Department
PreferencesQualifications For The Job
- Excellent communication skills, both oral and written and
ability to collaborate with other health professionals.
- Fluency in English, and preferably a second language, such as
Spanish or Chinese.
- Knowledge of the Nursing Process, the New York State Nurse
Practice Act and local, state and federal regulations.
- Ability to plan, problem solve, make decisions, set priorities
and take appropriate actions.
- Customer service-oriented behavior and attitude.
- Demonstrated leadership skills and ability to promote
change.Certification(s)/License(s)
- Licensed and currently registered to practice as a Registered
Professional Nurse in the state of New York.
- BCLS Certification required
- Certification in Ambulatory Care Nursing is
recommended.Educational LevelAssociates Degree in Nursing,
